Setting Appointments
Hours
Appointments can be made anytime after 8am Monday – Friday. If you are unable to keep a scheduled appointment please call us as soon as possible so we can assist you in rescheduling.
Changes or Cancellations
Twenty-four (24) hour notice is required for all patients cancelling or rescheduling office visits, new patient appointments, and consults. If our office does not receive twenty-four (24) hour notice a missed office visit/no show fee will apply to reactivate your record.
Children/Minors Policy
Patients under the age of 18 are required to be accompanied to their appointments by a parent or legal guardian. If a parent or legal guardian is unavailable, then the adult that is bringing the minor must bring a medical power of attorney to consent for evaluation and treatment authorized by the parent or legal guardian.
Do I Need A Referral?
Current referrals are necessary for ongoing care when your insurance requires it. You are responsible to have your PCP send a referral to our office, or have it on file with insurance.
It is the patient’s responsibility to confirm that your health insurance is contracted with Southwest Shoulder Elbow & Hand Center, PC. Agreements can change annually.
